We have helped many organizations by creating electronic forms from a standard paper form. Here is an e-form we did for a Community Action Agency in Maryland. This is for their LIHEAP department. Applicants click on a link on the CAP’s website and this electronic form comes up. The client fills it out, uploads required documents, signs it electronically, and hits submit.
The form is uploaded to Laserfiche and notifies their intake personnel that a new application has come in. Their case management system, empowOR, is also updated.
